Client Rights & Code of Conduct

At Life Balance Recovery, your safety, dignity, and well-being are our highest priority.
We follow the Utah Department of Health & Human Services (DHHS) Code of Conduct and CARF Behavioral Health standards to make sure every client receives ethical, respectful, and compassionate care.

Below is a simple, client-friendly version of your rights and what you can expect from us.


Your Rights as a Client

At Life Balance Recovery, you have the right to:

1. Be Treated with Respect

  • You will always be treated with dignity, kindness, and fairness.
  • We do not tolerate discrimination of any kind.

2. Feel Safe

You have the right to a safe environment that is free from:

  • Abuse
  • Neglect
  • Mistreatment
  • Exploitation
  • Retaliation
  • Violence or threats

Your safety is our responsibility.


3. Privacy & Confidentiality

  • Your personal information and records are protected by law.
  • We will explain how your information is used and who can access it.
  • You decide who we can share information with (unless required by law).

4. Participate in Your Treatment

You have the right to:

  • Be involved in decisions about your care
  • Ask questions about treatment
  • Receive information in clear, understandable language
  • Request changes or express concerns without fear of consequences

5. Understand Program Expectations

We will clearly explain:

  • What services we provide
  • What is expected of you in treatment
  • Which parts of the program are voluntary
  • Billing and payment information
  • Reasons you might be discharged and how to return if eligible

6. Communicate with Support People

Unless restricted by law or for your safety, you may:

  • Talk with family
  • Meet with clergy
  • Contact your attorney
  • Speak with your physician, counselor, or case manager

7. File a Complaint Without Fear

You can make a complaint at any time, for any reason, without retaliation.
You will never be punished, discharged, or treated differently for speaking up.


What You Can Expect from Us (Our Code of Conduct)

Every staff member at Life Balance Recovery is required to:

Treat you with respect and professionalism

We use trauma-informed, compassionate communication.

Maintain healthy boundaries

We never engage in inappropriate, abusive, or unprofessional behavior.

Protect your safety and well-being

You will never experience corporal punishment, humiliation, intimidation, or retaliation.

Maintain confidentiality

Your information is private and handled according to state and federal law.

Stay substance-free while providing services

Staff may not use alcohol or drugs while working.

Report safety concerns immediately

Any suspicion of abuse or neglect is reported right away to the proper agency.

Provide honest, accurate information

You will always receive truthful explanations about services, expectations, and fees.


How to File a Complaint

We take concerns seriously and encourage you to speak up if:

  • You feel unsafe
  • You feel your rights were violated
  • You were treated unfairly
  • You have concerns about staff behavior
  • You want to report anything that made you uncomfortable

Submit a Written Complaint (Preferred Method)

You may include your name or remain anonymous.


Other Ways to File a Complaint

  • Tell any staff member
  • Submit a written form in our office
  • Ask to speak with a supervisor or the Clinical Director
  • Place an anonymous written note in the secure complaint box (if applicable)

We will review your complaint within 1 business day and provide a response or update within 14 days, when appropriate.
